What is the 3×3 rule for implants in Turkey?

cerfs landing 300x94 (1)

3 days of soft food, 3 weeks of careful chewing, 3 months for bone fusion.

The‍‌‍‍‌‍‌‍‍‌ durability of an endosseous implant is generally fixed within the first several months of “loading” and recovery. To make a rather complicated physiological course of events more accessible, Professor Doctor Coşkun Yıldız uses the 3×3 Rule. The rule intends to strengthen the interface between the implant and the bone when it is weakest. International patients of Turkey who decide to invest in implant treatment are already halfway to success if they adhere to this protocol. It is a non-exhaustive yet definitive functional stability system that will definitely pay off for a lifetime.

Phase 1: The Critical 3 Days (Stabilization)

The first 72 hours are not more important as far as the formation of the clot and early infection prevention are concerned. At Lema Dental Clinic, we insist on sticking to these initial restrictions very strictly:

  • Thermal Regulation: Stay away from hot drinks. It has been shown that high temperatures cause vasodilation, leading to less clot formation or even re-bleeding that breaks down the protective fibrin scaffold.
  • Physical Restraint: Intense physical activity is prohibited (one exception only – sex). Physical exertion elevates your heart rate and thus blood pressure with the result of the increased risk or severity of the hematoma.
  • Dietary Consistency: Only cold, liquid nutrients are allowed. As long as the mechanical trauma to the sutured gingiva is prevented, you can have smoothies, yogurts, or cold soups.

Phase 2: The Transitional 3 Weeks (Soft Tissue Remodeling)

The soft tissue becomes the target of healing after the main inflammatory stage is over. Apart from Turkey, in the period when the gums are going through a remodeling process around the healing abutment, Dentist Polen Akkılıç and her team recommend the following:

  1. Soft Food Protocol: Continue with the soft-food diet. The seeds, nuts, and crusty bread should be avoided since they might get stuck in the surgical site or cause micro-movements of the titanium post.
  2. Hygiene Calibration: Start with gentle cleaning. Although the wound must not be traumatized directly, the rest of the mouth should be kept sterile to prevent bacteria from migrating.
  3. No Suction Force: Do not use straws or smoke. The vacuum pressure developed in this way can cause plant disturbance of vascularization that has just started beneath the surface of the oral mucosa.

Phase 3: The 3-Month Window (Secondary Stability)

The third “3” points to the change of primary (mechanical) into secondary (biological) stability. Professor Doctor Coşkun Yıldız mentions that the process of osseointegration, which is the actual binding of bone to titanium, reaches its maximum effectiveness after about 12 weeks. This period is when the implant is considered by us “safe” for the final prosthetic loading in Turkey. If forced at any time before this window is closed, the implant may become encapsulated in fibrous tissue instead of being bonded to the bone and thus will ‍‌‍‍‌‍‌‍‍‌fail.

FAQ: Navigating the 3×3 Rule at Lema Dental Clinic

Why is the 3-week mark so important for the “soft diet”?

Dentist Polen Akkılıç and her team explain that around the third week, the “primary stability” (the grip of the screw) slightly dips before the “secondary stability” (bone growth) fully takes over. This is the period of highest risk for micro-movements.

Can I travel back from Turkey during the 3×3 period?

Yes. Most patients return home after the first 3 to 7 days. However, the 3-week and 3-month rules must be strictly followed at home to ensure the success of the treatment initiated at Lema Dental Clinic.

What happens if I accidentally eat something hard during the first 3 weeks?

A single incident might not cause failure, but you should monitor for pain or mobility. Professor Doctor Coşkun Yıldız recommends contacting your surgeon in Turkey immediately if you feel any shifting in the implant area.

Is 3 months enough for everyone?

Usually, yes. However, for patients who require extensive bone grafting, the 3-month rule might be extended to 6 months. At Lema Dental Clinic, we use 3D imaging to confirm that the bone is ready before moving to the next phase.

Does the 3×3 rule apply to All-on-4 implants, too?

Absolutely. While All-on-4 allows for “immediate loading,” the 3×3 rules for diet and hygiene are even more critical because the implants are interconnected and sharing a load during the healing phase.
