Yes, clear aligners can fix severely crooked teeth with expert planning.
Every single day, we have people coming into the clinic with their smiles hidden. They come in, point at their overlapping teeth or their misaligned bite, and then ask that hopeful question: Is it possible for me to fix this without metal braces? Certainly, the clear aligners have made huge progress in the last decade. However, fixing extremely crooked teeth can’t be done by simply wearing clear plastic. It requires careful planning and top skills.
Here at Lema Dental Clinic in Turkey, we treat all kinds of smiles. We can remove minor imperfections as well as resolve major bite problems. The truth is, the success of clear aligners lies 100% with the doctor who is leading your treatment.
What Is Our Method to Move “Stubborn” Teeth?

Imagine that your teeth are like mature trees that have their roots deeply embedded in the jawbone. You cannot merely uproot a tree and move it elsewhere. You have to educate the roots through the soil gently, which allows the bone to heal after the movement.
To begin with, very crooked teeth will need to be untwisted. Occasionally, they also need to be tilted a bit downwards. Clear plastic trays cannot always hold round teeth firmly enough to pull this off.
This is the picture we get in the clinic: to get the above-mentioned teeth to move, we use help from “attachments”. These are very small, tooth-colored bumps that are bonded onto your teeth. In fact, they are little handles that provide the aligner with a solid point of pressure.
Prof. Dr. Coşkun Yıldız always says, “The clear tray is just the car. The small attachments on the teeth act as the engine. They decide which tooth moves first to make room for the rest.”
Mild vs. Severe Cases: What to Expect
We created a quick guide for you to understand how we treat different smiles. Have a look at what aligner treatment can do for you, depending on your teeth:
| Tooth Spacing | Can Aligners Fix It? | Average Time | Extra Tools Needed |
| Slight Crowding (1-3mm) | Highly Predictable | 4 to 8 Months | Few to no attachments. |
| Moderate Crowding (4-6mm) | Very Predictable | 9 to 15 Months | Standard attachments, gentle space-making. |
| Severe Crowding (7mm+) | Possible with strict planning | 18 to 24+ Months | Many attachments, rubber bands, or possible extractions. |
| Severe Twists (>15 degrees) | Challenging but viable | 14 to 20 Months | Special attachment designs, careful staging. |
The Lema Dental Clinic Method

There are three main ingredients to fixing a complex smile: the right team, the right tools, and the right technique.
Dentist Polen Akkılıç and her dental team use intelligent 3D scanning to create a digital replica of the top of your teeth, roots, and jaw bones.
We have to admit that sometimes the jaw simply doesn’t have space for all the teeth. If that were the case, earlier doctors would have only extracted teeth. These days, however, we tend to think of aligners as a way to widen a smile, and none of those tough, irreversible options are required. In some cases, we may also administer minimal polishing of the teeth that creates just enough space to reduce crowding without tooth extraction.
We definitely don’t give you a smile-aligner box and a “good luck” and wave you goodbye. We are fully aware that complex smiles need careful monitoring. It is essential to us that the roots of teeth move safely along, too. So, if you plan on coming to Turkey for your treatment, you will be using world-class equipment and real experts who deal with hard cases on a daily basis.
Patient FAQs: Straight Answers from the Clinic
Not really, but it could happen. If your jaw is too tiny, trying to cram the teeth into space can actually shove them out of their bone. This results in gum problems. We want to keep your natural teeth as much as possible. But if one tooth removal will give us a better, safer bite, we will definitely explain it to you and provide the guidance.
This is a serious undertaking; not something you’ve signed up for a short period only. Typically, extreme cases will have a treatment duration of 18 to 24 months. Rapid movements could adversely impact the roots. Going slow is the method of choice to get you a safe, long-lasting result.
My term for that is “pressure.” When starting with a new tray, for 1 or 2 days, the teeth will feel a bit tight. That is the sign that the aligner is doing its job. Since clear aligners make tooth movements only in very small increments, the majority of patients even consider them more comfortable than metal braces.
No way. Mail-order aligners will only do magic for the front teeth. Also, they do not come with attachments. Trying to fix a serious bite without the use of X-rays and the doctor monitoring your progress is very dangerous. It’s like building a house without first checking the ground. In most cases, it results in a spoiled bite.
At Lema Dental Clinic, you will get lavish care and the very best in medical science. Being a dental hub in the world, Turkey boasts highly experienced doctors and the latest 3D technologies. Besides, you will be receiving excellent, thoroughly monitored results in a picturesque setting.
